What
Our Independent Living Staff will meet with a consumer to evaluate Independent Living (IL) needs and provide support in developing an Independent Living Service Plan (ILSP)

Common Independent Living needs Include, but are not limited to:

  • Access (ramp, bathroom, kitchen, accessible housing, stairlifts)
  • Assistive technology (could be all sorts of things)
  • How to fill out forms/applications
  • How to contact and access a service (VR, CAP, Maximus, etc)
  • Personal assistance (home care) supports and services
  • Driving or adaptive vehicle needs
  • Mobility equipment (walker, cane, scooter, power or manual wheelchair)
  • Learn how to access public transportation
  • Home care equipment needs (commode, shower chair, raised toilet seat, etc)
  • Communication (interpreter, technology, etc)
  • Information technology (tablet, smart phone, computer, software)
  • Educational or vocational goals
  • Self Advocacy
  • Community participation (community/recreational activities, peer services)
  • Finding providers
  • Budgeting and money management
  • Funding for home access or adaptive equipment
  • and more . . .

Where
Alpha One Independent Living Staff (ILS) will meet in person with a consumer at their home, workplace, school, or in the community or by virtual meeting to complete an IL evaluation and service plan.   The consumer makes the choice of preferred means and location.

Follow-Up
After completion of the ILSP, Alpha One staff will research and provide additional resources and information as necessary or helpful in accordance with the Plan.

If additional support is needed to meet an identified goal on the ILSP, Alpha One staff will meet 1:1 to provide Independent Living Skills Training

  • The consumer is in control in deciding what goals they would like to work on with Skills Training support
  • The number of Skills Training visits varies depending on the specified goal, and ends when the goal has been met or the consumer elects to stop pursuing.

If you have a disability that impairs your ability to function independently at home or in the community, you are eligible for these Independent Living services
